② 내적욕구의 증가에 따라 지나치게 활동하는 경우 정신운동성 항진 또는 흥분이라고 한다. 단순하게는 잘 구별되지 않는 소음들에서 뚜렷한 내용이 있는 특정한 사람의 말소리가 들리는 것까지 그 내용이 다양하다.

착각은 실제로 받아들여진 외부 자극이 감각기관에서 뇌의 적절한 부위에 전달되어 해석되는 과정에서 잘못 인식되는 현상을 말한다. 대부분의 경우 불쾌하고 경멸적이고 추잡하거나 또는 비난하는 내용으로 그의 망상과 밀접한 관계를 가지고 나타나는 수가 많다. 망상과 마찬가지로 환청도 환자의 내면이 투사된 것이므로 그 외 감정이나 그 밖의 전체적인 심리를 반영한다. 독성물질의 섭취나 또는 열성질환에 의해 일어나는 중독성 착란상태에서의 착각은 맑은 의식상태에서 일어나는 착각보다는 심리적인 의미가 적다. 착각은 환각과 동일한 심리적 기능을 가지고 있긴 하나 환각보다는 현실 왜곡의 정도가 덜하다. 조울병의 경조증 또는 조증환자에게서 흔히 볼 수 있다.
