) Diagnosis
(1) Jugular venous pressure
Jugular venous pressure의 상승prominent X descent : atrial relaxation
abscent diastolic Y descent : atrial emptying
(2) EKG
electrical alterance(가장 특징적)low voltage of QRS
nonspecific ST-T change
(3) Chest X-ray
심장이 구형 또는 물주머니(water bottle) 모양pulmonary congestion은 드물다
(4) Echocardiography
가장 도움이 되는 검사large pericardial effusion
right atrium collapse, diastole시 right ventricle collapse
(5) Catheterization
`equalization of pressure` : 상승되어 같아짐RA = DRV = DPA = PCWP = LA = DLV(D=확장기말)
6) Treatment
Cardiac tamponade 치료의 기본은 심낭강의 배액(pericardial drainage)
수축기 혈압 낮을때에는 수액투여
이뇨제는 사용하지 않음
(1) Pericardiocentesis(심막천자)
일시적인 방법으로 심장초음파검사하에 시행하는 것이 안전
